Requirements
- Identifying as a transgender or non-binary person.
- Submit financial need
- Born and reside in Puerto Rico.
- Born in the United States and reside in Puerto Rico
- Born in Puerto Rico and lives in the United States
Cost
- Legal service: Free
- Filing fees: $78.00
- Stamp: $5.00
Documents
For people over 21 years of age who were born and reside in Puerto Rico:
- Birth certificate
- Negative criminal record certification with original name
- Negative criminal record certification with the proposed name
- Certification of no debt from ASUME or certification of payment plan
- Certification of no debt with the Treasury Department or payment plan certification
- Certification of no debt with the CRIM or payment plan certification
- Negative bankruptcy certification

To start the legal name change process, you must complete the following form in its entirety. Once the form is completed and the documents are received, we will be communicating by phone to establish the next steps to complete the process.
